Flight Crew Licensing
Listed below are the Bermuda Department of Civil Aviation Flight Crew Licensing Forms. We have provided them in Adobe PDF format. You will need the latest version of Adobe Acrobat Reader to view, fill in, and print the form. We highly recommended filling out the forms using Acrobat Reader, once you have finished typing in your information on the form, print it off, sign it and fax it back to the Department of Civil Aviation at (441) 293-2417 and/or (441) 293-0783.

| Bermuda Aviation Legislation Test A test on Bermuda aviation legislation will be introduced during 2007. At the present time it is not a requirement for the issue of a Certificate of Validation. Until the test is available, applications may be submitted without this requirement being satisfied. | ||
