How to Verify your FAA Certificate

Listed below are the Bermuda Department of Civil Aviation Flight Crew Licensing Forms. We have provided them in Adobe PDF format. You will need the latest version of Adobe Acrobat Reader to view, fill in, and print the form. We highly recommended filling out the forms using Acrobat Reader, once you have finished typing in your information on the form, print it off, sign it and fax it back to the Department of Civil Aviation at (441) 293-2417 and/or (441) 293-0783.

If you have any problems or difficulties with the forms, please contact the BDCA as quickly as possible.
